- As a follow-up to all emergencies, non-emergency accidents or incidents; an Accident & Incident Report needs to be completed.
- The form can be found at Online Requests, Forms & Documents
Accident & Incident Reports completed by
- the person injured or affected (if possible)
- the person reporting the accident & incident
- the person in charge at the scene
- the affected person’s immediate supervisor
Copies of the form are distributed to
- Security (2E.14)
- Director of Facilities Management (2E.21)
- Director of Student Services (4E.2)
